Home-based: Family Support Caseworker

Job Overview:

The Family Support Caseworker will provide services to pregnant women and families with newborns and children up to five years old to people with refugee, SIV, Asylee, and Humanitarian Parolee status. The caseworker will coordinate services to ensure children and their families are provided with assistance, resources, and integrate into their new community. Monthly field trips will be coordinated and held including various health, public benefits, parenting, and children in-language workshops to provide families with wrap-around services and information. This position will report directly to the Health and Wellness Coordinator in the Safety and Wellness Department.

Major Responsibilities:

  • Screen for eligible clients based on their immigration status and length of time in the U.S., successfully enrolling eligible clients into the program.
  • Work with pregnant women, children, and their families, identifying needs, creating service plans, ensuring progress through individual service plans, and coordinate referrals as needed.
  • Continually assess ongoing changes in behavior and general conditions that could impact the safety of a participant.
  • Provide services with public benefits, school enrollment, vaccinations, health appointments, direct assistance, and other areas where relevant.
  • Build and maintain relationships with community service providers in relevant areas.
  • Coordinate with the Health and Wellness Coordinator monthly field trips and wellness/informational workshops with other participating grantees.
  • Travel regularly throughout the service delivery area (home visits, agency visits, service provider visits, field trips, etc.)
  • Maintain case file documentation, including clear and concise case notes and maintain internal tracking systems and data collection.
  • Report regularly on progress of cases, assisting in monthly data collection and reports.
  • Participate in all program meetings, staff development activities, trauma-informed care trainings, and fully engages as a member of the team.
  • Comply with all policies, procedures, and protocols of the agency.
  • Other related duties as assigned.

Job Requirements:

  • Prefer minimum education of a bachelor’s degree or certificates in social work, medical training, child development, education, psychology, sociology or other behavioral science equivalent;
  • Relevant professional experience in human services including provision of direct services to underserved communities; minimum 1-2 years of experience working with children, families, and/or child welfare;
  • Previous multi-cultural experience and demonstrated ability to communicate well with people of varied backgrounds strongly preferred;
  • Comfortability collaborating with governmental agencies and community providers;
  • Excellent communication skills, with fluence in written and spoken English;
  • Ability to coordinate events and workshops individually and within a team with excellent problem solving skills;
  • Proficiency in one or more other languages strongly desired;
  • Proficient in Microsoft Office Applications (Word, Excel, Outlook) and strong keyboarding skills and internet usage and research skills;
  • Prior experience using data and tracking systems strongly preferred;
  • Attention to detail and ability to organize and track large amounts of information efficiently and accurately;
  • Commitment to teamwork and collaboration across all of IRC’s programs;
  • Valid driver’s license, reliable vehicle with up-to-date insurance, and the ability to travel regularly throughout the service delivery area.

Working Environment:

  • A combination of standard office environment and remote work;
  • ‘Field’ time within the service delivery area to perform the above-outlined responsibilities;
  • Offices hours are scheduled as Monday-Friday, 9am-5pm; may require some evening and weekend work
