Case Management Global Forms

The Global Case Management Forms are a package of forms developed to facilitate standardized data collection processes that enable program staff to aggregate (or disaggregate) data to track trends. The forms promote standardized methods of information gathering in a particular context that will facilitate improved information sharing, referrals, and data analysis across agencies. The Global Case Management Forms can also assist in reducing the workload in developing new forms each time case management systems/interventions are established. While some data fields are defined, forms should be contextualized to each country in consideration to culture, language and local priorities for case management program needs.
